They go off of popularity and ratings then they practice with each other in the ring. Then when it comes time to wrestle they go at it, but depending on the time of the show is to how long the match will last. The Ref is actually the one who tells them when to end the match. Watch real close and look in the ear of the Ref there is an ear piece that the people in the back are talking to him. They give him the time and he decides when to call the match.

For TV they will either a)let the heel (bad guy) really screw over the good guy so the feud culminates with the good guy winning at the pay per view, or b) let the good guy win on TV and lose at the Pay per view, & have the heel cheat to win, just to inflame the audience even further. Then the feud will spill over until the next PPV. Once in a while they'll let belts change hands on TV just to keep the fans interested but the big feuds are almost always settled on pay per views. It's all about working the audience's emotions to maximize viewership and pay per view incomes. They have a staff of writers that carefully construct all the storylines, with the occasional twist and turn thrown in for good measure. So that's basically how the winners are picked.
